
New Student Enrollment The following information must be completed before your child can be scheduled for the entrance exam. 1. Application & Testing Fee - $25 2. Last two years final Report Card and TAKS and/or Achievement Testing Results

4. Birth Certificate 5. Social Security Card 6. Medical Emergency Release Form (both sides). 7. *K-3 & K-4 only - State licensing medical screening form ( Must have before enrollment is completed) 8. *K-3 & K-4 only - Student/Parent Handbook Agreement Form completed. Once your child has been accepted the following must be completed at enrollment: 1. Parent/School Agreement 2. Email Address ( Mandatory*, If not available, see Accounting Dept.) 3. Tuition Contract 4. Enrollment Fee of $225.00 per student - includes student insurance, standardized test, student ID badge, FCA yearbook, student agenda (Fee is Non-refundable) The following payments are due by August 15th: 1. Supply Kit Fee (mandatory for K3, K4, and K5 - optional for 1st -5th) 2. Book Fees - include text books, SAT workbook, student learning boards $155 - per K3 -K4 student $255 - per K5 $325 - per 1st - 5th grade student $350 - per 6th - 8th grade student $400 - per 9th - 10th grade student $450 - per 11th - 12th grade student School Enhancement Fee (Option selected on tuition contract) $500 per family (due April 1, 2011) Acceptance is based upon previous educational records, behavioral patterns, academic testing, spiritual pursuits and personal interviews with the parents and/or student.
